Start from the record, not from memory
The nurse is already signed in, and the resident list comes from the connected care system. Handover never begins by repeating credentials or asking the nurse to reconstruct a list from memory.
For each resident requiring handover, the assistant presents a summary prepared from the available record and reads it aloud while the outgoing nurse follows the same text on screen. The focus stays on residents with significant changes, using the care team's agreed criteria.

Review, correct, and verify
Each summary brings forward the significant change, relevant recent care, outstanding actions, and the information the next nurse needs. The outgoing nurse checks the account, corrects inaccuracies, and adds anything material that is missing.
The assistant then presents the corrected content for verification. A draft is always distinguished from a nurse-verified summary, keeping the nurse's review visible in the handover process.
